For the benefit and convenience, the Village of Pleasant Hill is hereby authorized to automatically deduct Automatic Clearing House (ACH) from my checking account the amount of my utility bill every two months (Jan, Mar, May, July, Sept and Nov) on the 15th day of the month or the first business day following (not the due date stated on the bill). I agree to be bound by the rules set forth below, in addition to the bank’s regular rules governing checking/savings accounts.
Rules
1. The term “bill’ means, Water, Sewer, Refuse, Capital Water and Capital Sewer.
2. Depositor agrees that the bank’s right with respect to each bill shall be the same as if each bill were a check drawn on the bank account and signed personally by the depositor and that the bank be fully protected in honoring each bill.
3. This authority is to remain in effect until revoked by the bank, Village (utility business) or depositor. If the depositor account does not contain sufficient funds twice in one year, the utility business will void this agreement with written notice to the customer.
4. The Bank’s sole responsibility and duty shall be to charge the depositor’s account for the amount issued from the utility company.
5. The Bank is under no obligation to notify depositor of either payment or dishonor of utility bills. Bills will be returned unpaid to the utility company if depositor’s account does not contain sufficient funds to cover the amount of said bill. If payment is denied by the bank, the customer will be notified and will be charged a $30.00 processing fee and be responsible to make a payment by check, cash, or money order before the due date to avoid penalties.
